Where can I obtain the Notification of Health Insurance Dependent (Change)? How do I fill it out?

The fastest way to get a Notification of Health Insurance Dependent (Change) is to download it from the IBM Japan Health Insurance Association website. (Go to the Notices/Application Forms page.) The forms are in PDF format. Open them in Acrobat Reader and print them out (on A4-sized paper) to use them. Sample completed forms are available from the links next to those for the blank forms.
