Childbirth

The Childbirth and Childcare Lump-Sum Grant is paid when an insured person gives birth. The Dependents’ Childbirth and Childcare Lump-Sum Grant is paid when a dependent family member gives birth. In addition, the newborn must be added as a dependent.

Adding the child as a dependent

Adding a baby as a dependent requires certification by the IBM Japan Health Insurance Association.
Download the documents indicated below and submit the application form to the address shown on the upper left. When applying to add a newborn baby as a dependent, you do not need to attach documentation certifying birth. Simply submit the application form (original and copy).
If both the husband and wife are employed, the baby will become a dependent of the spouse with the higher income.

Requesting the Childbirth and Childcare Lump-Sum Grant

Choose one of the three methods A, B, or C below.

A. Using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

Submit the documents on the agreement to use the system to the medical care institution or other facility where you plan to give birth. (You do not need to undertake any procedures with the Health Insurance Association. For more information, check with the medical care institution or other facility where you plan to give birth.)

Note that this system is not available for some hospitals and other facilities.

Caution
  • You cannot use the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions in the following cases:
    • When you plan to give birth overseas
    • When using the system of loans to cover the costs of childbirth
    In the above cases, apply using the Application for Childbirth and Childcare Lump-Sum Grant.
  • You are not eligible to receive duplicate benefits from other health insurance plans (including National Health Insurance and the Japan Health Insurance Association).
    If you plan to use the IBM Japan Health Insurance Association’s system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions after forfeiture of eligibility under the Association, you will be issued a Certificate of Loss of Eligibility for Health Insurance. Submit this Certificate to the hospital or other facility.

B. Using the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

If you would like to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf, submit the following application to the Health Insurance Association in advance.

Required documents
Copy of maternal and child health handbook (page showing name of person performing delivery and expected date of birth)
Deadline Within two months of the expected date of birth
Applies to Insured persons and dependents with expected childbirth dates up to two months away, who wish to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on their behalf

Caution
  • You cannot use the system of direct receipt of the Childbirth and Childcare Lump-sum Grant by the medical institutions on your behalf in the following cases:
    • When the medical care institution or other facility where you plan to give birth does not agree to receive the grant on your behalf
    • When you plan to give birth overseas
    • When using the system of loans to cover the costs of childbirth
    In the above cases, apply using the Application for Childbirth and Childcare Lump-Sum Grant.
  • Apply within two months of the expected date of birth after obtaining the consent of the medical care institution or other facility where you plan to give birth to receive the grant on your behalf.
  • The relevant signatures/seals must be affixed in the spaces for the insured person ⑫ and recipient of the grant on your behalf ⑬ on the Application for Payment of Childbirth and Childcare Lump-sum Grant (Advance Application).
  • You are not eligible to receive duplicate benefits from other health insurance plans (including National Health Insurance and the Japan Health Insurance Association).
    If you plan to give birth within six months after forfeiture of eligibility under another health insurance association or other insurer under which you were covered previously, or within six months after forfeiture of eligibility under the Association, be sure to fill out completely and sign/seal spaces ⑩–⑪ on the application form.
  • Enter a circle in the appropriate part of space ⑨ on the application form to indicate whether or not the institution you plan to use is a member of the maternity medical care compensation scheme.

C. When you have paid the entire amount of childbirth costs to the medical facility

If you have not used the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf, or you have given birth overseas, submit the following application to the Health Insurance Association.

Required documents

Attachments required:

  • Copies of the agreement documents from the medical care institution or other facility
    (Showing the fact that you have not concluded a proxy agreement with the medical care institution or other facility on use of the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or similar agreement and showing the name of the Health Insurance Association as the application recipient.)
  • Copies of receipts for and details of childbirth costs
    (Showing the fact that you have not concluded a proxy agreement with the medical care institution or other facility on use of the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or similar agreement and, if giving birth under the medical administration of an institution that is a member of the maternity medical care compensation scheme, affixed with a stamp certifying that fact)
Deadline At the earliest possible date after the birth
Applies to Insured persons or dependents who have paid the entire amount of childbirth costs at the medical facility without using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on their behalf

Caution
  • The doctor, midwife, or other person handling the birth must fill out and affix his or her seal to the space for certification by the doctor, midwife, etc. ⑮–⑱ on the Application for Payment of Childbirth and Childcare Lump-sum Grant.
    Documents certifying the birth or similar documentation are not acceptable for this purpose.
  • You are not eligible to receive duplicate benefits from other health insurance plans (including National Health Insurance and the Japan Health Insurance Association).
    If you plan to give birth within six months after forfeiture of eligibility under another health insurance association or other insurer under which you were covered previously, or within six months after forfeiture of eligibility under the Association, be sure to fill out completely and sign/seal spaces ⑬–⑭ on the application form.
  • Enter a circle in the appropriate part of space ⑨ on the application form to indicate whether or not the institution you plan to use is a member of the maternity medical care compensation scheme.

Loans to cover the costs of childbirth

Required documents
  • Contact the IBM Japan Health Insurance Association to request a loan application form.
Applies to

An insured person who expects to receive payment of the Childbirth and Childcare Lump-sum Grant or similar benefits and meets the following conditions:

  • Plans to give birth within one month or has a dependent who plans to give birth within one month
  • Has been pregnant for at least four months or has a dependent who has been pregnant for at least four months, and has a temporary need to make a payment to the medical care institution.

Note: This system cannot be used together with the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf.

Loan amount

Up to 336,000 yen (interest-free)
Up to 323,200 yen for a birth planned to take place at a facility that is not a member of the maternity medical care compensation scheme.

  • Note: Amounts above correspond to 80% of the amount of the Childbirth and Childcare Lump-sum Grant (statutory benefit) of 420,000 yen (404,000 yen for a birth at a facility that is not a member of the maternity medical care compensation scheme).
Period of loan Until the date of payment of the Childbirth and Childcare Lump-sum Grant corresponding to the loan
