If you lose your health insurance card

Your health insurance card is an important identification document. Take the following steps at the earliest possible date if you lose or damage your health insurance card. (Notify the nearest police station if lose your card.)
If you later find your old health insurance card (after being issued a new one), return the old card to the Health Insurance Association.
The same procedures apply if you lose your Elderly Benefits Card or Certificate of Application of Maximum Copayment Amount.

If you lose your health insurance card

Procedures

  1. STEP1Apply to have your health insurance card reissued
    If you lose your health insurance card, download the documents specified below and submit the application form to the address shown on the upper left.
  2. STEP2Receive your reissued health insurance card
    Your new health insurance card will be sent through your employer (or mailed to your home if you are no longer employed).
    Safeguard your new health insurance card.
Required documents

<Identification Documents>
You may be asked to attach the following documents or document copies to verify the applicant’s identity.

  • An identification document showing a facial photograph of the applicant (e.g., driver’s license, passport). If the applicant does not have picture ID, an official certificate such as a certificate of residence.
  • If somebody else is applying on your behalf, documentation of the relationship between you and the person applying, in addition to the ID of the person applying, etc.
Deadline At the earliest possible date
Applies to Insured persons and dependents who have lost their health insurance cards
