  • * Use the form on the "Web Inquiries" page to obtain the application form for months of treatment received in or before September 2024.

    • * Required for the first time you apply and every six months thereafter (if treatment continues)
    • * Required monthly for manual correction of deformities
  • Report on treatment (copy) (when obtaining subsequent approval)

  • The payment decision will be made after a review by the Health Insurance Association.
  • We may make inquiries with the patient and medical care institution regarding the nature of the treatment and the cause of injury.
  • The payment decision will be made three or more months after the month of treatment to confirm the consultation status with the medical care institution.
  • Acupuncture and moxibustion are not eligible for health insurance benefits if receiving treatment at a medical care institution for the same illness or injury. (The term "treatment" here does not refer to exams, tests, or issuance of a letter of approval.)
  • As a result of the review, some or all of the benefit may be withheld.
