1. Flow of procedures for withdrawal upon becoming an insured person of another health insurance association due to re-employment

Submit the following documents to withdraw from the Voluntarily and Continuously Insured Person/Special-Case Retired Persons Medical Care System due to re-employment.
* Submit all documents together after obtaining the Notice of Eligibility Information or Health Insurance Eligibility Certificate from your new health insurer.

• Required: Forfeiture Notice for Voluntarily and Continuously Insured Persons and Special-Case Retired Insured Persons / Premium Refund Request
• Required: Copy of Notice of Eligibility Information or Health Insurance Eligibility Certificate from your new health insurer
• If applicable: Valid Health Insurance Eligibility Certificate(s) issued by the IBM Japan Health Insurance Association (including those for any dependents who were Association members)

4. Flow of procedures for withdrawal from the Voluntarily and Continuously Insured Person/Special-Case Retired Persons Medical Care System due to application from the insured person in question

• The Voluntarily and Continuously Insured Person/Special-Case Retired Insured Person should complete a Forfeiture Notice for Voluntarily and Continuously Insured Persons and Special-Case Retired Insured Persons/Premium Refund Request and send it to the Health Insurance Association.
• The Health Insurance Association will process the cancelation of eligibility, the effective date of which will be the first day of the month after the month that includes the date on which the Association receives the Forfeiture Notice for Voluntarily and Continuously Insured Persons and Special-Case Retired Insured Persons/Premium Refund Request.
• The Health Insurance Association will send a Forfeiture Notice Due to Application to the Voluntarily and Continuously Insured Person/Special-Case Retired Insured Person.
  (A Certificate of Loss of Eligibility will be enclosed for Special-Case Retired Insured Persons using My Number Cards as Health Insurance Certificates.)
• Voluntarily and Continuously Insured Persons/Special-Case Retired Insured Persons with valid Health Insurance Eligibility Certificate(s) issued by the IBM Japan Health Insurance Association should return the certificate(s) by the [Deadline for Return of Health Insurance Eligibility Certificate(s)] indicated on the Forfeiture Notice Due to Application.
• At the same time it collects the valid Health Insurance Eligibility Certificate(s), the Health Insurance Association will issue a Certificate of Loss of Health Insurance Eligibility and send it to the home address of the Voluntarily and Continuously Insured Person/Special-Case Retired Person who applied for withdrawal.
