Flows of procedures for withdrawal from the Voluntarily and Continuously Insured Person Medical Care System due to 1. re-employment and 4. application from the insured person in question

1. Flow of procedures for withdrawal upon becoming an insured person of another health insurance society due to re-employment

Submit the following three documents to withdraw from the Voluntarily and Continuously Insured Person Medical Care System due to re-employment:
* Submit all three together after you receive your new health insurance card.

4. Flow of procedures for withdrawal from the Voluntarily and Continuously Insured Person/Special-Case Retired Persons Medical Care System due to application from the insured person in question

  • The Voluntarily and Continuously Insured Person/Special-Case Retired Insured Person should complete a Forfeiture Notice for Voluntarily and Continuously Insured Persons and Special-Case Retired Insured Persons/Premium Refund Request and send it to the Health Insurance Association.
  • The Health Insurance Association will process the cancelation of eligibility, the effective date of which will be the first day of the month after the month that includes the date on which the Association receives the Forfeiture Notice for Voluntarily and Continuously Insured Persons and Special-Case Retired Insured Persons/Premium Refund Request.
  • The Association will send a Request to Return Health Insurance Card Upon Receipt of Forfeiture Notice for Voluntarily and Continuously Insured Persons and Special-Case Retired Insured Persons to the Voluntarily and Continuously Insured Person/Special-Case Retired Insured Person.
  • We ask that the Voluntarily and Continuously Insured Person/Special-Case Retired Insured Person return the health insurance card(s) to the Association by the [Deadline for Return of Health Insurance Card] indicated on the Request to Return Health Insurance Card Upon Receipt of Forfeiture Notice for Voluntarily and Continuously Insured Persons and Special-Case Retired Insured Persons.
  • Upon receiving the health insurance card(s), the Association will issue a Certificate of Loss of Eligibility for Health Insurance and send it to the home address provided by the withdrawn Voluntarily and Continuously Insured Person/Special-Case Retired Insured Person.
