I was injured while I was traveling. I didn't have my health insurance card with me, so I paid the entire amount required for treatment in cash. How do I submit a claim to be reimbursed for these costs?

Use the Application Form for Medical Care Expenses to the Health Insurance Association (HZD-YY1). Provide a detailed statement of the medical care (original) from the medical care institution and submit with your receipt(s) (original). The Association will review these documents and pay benefits according to its standards.
