Submit the Application Form for Overseas Medical Care Expenses with the attending physician's statement and original itemized receipts attached (or a photocopy of the credit-card statement if you paid by credit card) to the Health Insurance Association (HZD-YY1). The amount paid will be calculated based on medical care costs for the equivalent care in Japan. Since the Application Form for Overseas Medical Care Expenses is a five-sheet carbon-copy form, it is not available for download. Request it from the Health Insurance Association.
