Where can I obtain an application form?

Use the Web inquiry service on the IBM Japan Health Insurance Association website to request to have an Application Form for Overseas Medical Care Expenses sent to you. The form will be sent to your place of work in Japan or to your residence address in Japan. It will not be sent to your residence overseas.
