My wife worked through June. Her income this year exceeds the dependent standard amount of 1.3 million yen. Is she no longer a dependent?

The Health Insurance Association authorizes dependents depending on whether their income after leaving employment (including non-employment income, such as real-estate income) exceeds the standard amount established by law (1.3 million yen/year if under age 60; 1.8 million yen/year if disabled or age 60 or above). For this reason, even if your wife's salary income while working over the months of January through June exceeded 1.3 million yen (or 1.8 million yen if aged 60 or above), it will not affect her eligibility as a dependent.
