What should I do if I need to see a doctor suddenly on my way home from work but I don't have my health insurance card with me?

You can receive insurance treatment even if you don't have your health insurance card with you. While you will need to pay the medical care costs up front, settlement methods vary by medical care institution. Be sure to check on handling of insurance treatment at the counter of the institution before receiving care. If the institution asks you to show it your health insurance card at a later date, do so. If the medical care institution does not provide settlement but requires you to pay the entire amount up front, claim benefits by submitting the Application Form for Medical Care Expenses to the Health Insurance Association (HZD-YY1) at a later date with a detailed statement of medical care (original) and your receipt(s) (original) attached. The Association will review these documents and pay benefits according to its standards.
