What documents do I need to attach to the application form?

Attach the following documents to the application form:

(1) Certificate of residence (photocopies are invalid): for insured person and dependents (must show relationship)

(2) National pension/employee's pension certificate (submit one photocopy; you can obtain a pension certificate about two months after claiming pension benefits; if you cannot submit it when applying, send it later after you receive it.)

(3) Certificate of Loss of Eligibility for Health Insurance (if a member of an insurance plan other than the IBM Japan Health Insurance Association; not required from members of National Health Insurance)

(4) Photocopy of National Health Insurance card (if a member of National Health Insurance)

(5) If applying for a dependent transferring from another health insurance association or National Health Insurance, the documents shown on the List of Documents Required for Certification of Dependents. If you will continue to be a member of the IBM Japan Health Insurance Association, submit only the documents under (1) and (2) above.
