Childbirth

The Childbirth and Childcare Lump-Sum Grant is paid when an insured person gives birth. The Dependents’ Childbirth and Childcare Lump-Sum Grant is paid when a dependent family member gives birth. In addition, the newborn must be added as a dependent.

Request a Childbirth and Childcare Lump-Sum Grant

A. When using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

Submit the documents on agreement to use the system to the medical care institution or other facility where you plan to give birth. (You need not take any action with respect to the Health Insurance Association. For details, check with the medical care institution or other facility where you plan to give birth.)
Even if you use this system, you must apply to the Health Insurance Association to receive additional benefits.

Caution
  1. You cannot use the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions if you plan to give birth overseas.
  2. If the amount paid is less than the statutory benefit amount, the difference will be paid automatically via your employer about three months after the month of childbirth. (For those covered by the system for Voluntarily and Continuously Insured Persons or the Special-Case Retired Persons Medical Care System, the difference will be transferred to the account you registered when joining the system.)

B. When using the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

If you would like to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf, submit the following application to the Health Insurance Association in advance.

Required documents
Copy of maternal and child health handbook (page showing name of person performing delivery and expected date of birth)
Deadline Within two months of the expected date of birth
Applies to Insured persons and dependents expecting childbirth up to two months in the future, who wish to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on their behalf

Caution
  1. You cannot use the system of direct receipt of the Childbirth and Childcare Lump-sum Grant by the medical institutions on your behalf in the following cases:
    a.When the medical care institution or other facility where you plan to give birth does not agree to receive the grant on your behalf
    b.When you plan to give birth overseas
    In the above cases, apply using the Application for Childbirth and Childcare Lump-Sum Grant.
  2. Apply within two months of the expected date of birth after obtaining the consent of the medical care institution or other facility where you plan to give birth to receive the grant on your behalf.
  3. The spaces for the insured person and recipient of the grant on your behalf must be filled out/sealed on the Application for Payment of Childbirth and Childcare Lump-sum Grant (Advance Application).
  4. You are not eligible to receive duplicate benefits from other health insurance plans (including National Health Insurance and the Japan Health Insurance Association).
    If you plan to give birth within six months after loss of eligibility under another health insurance association or other insurer under which you were covered previously, or within six months after loss of eligibility under the Association, be sure to fill out completely and sign/seal spaces ⑧-⑨ on the application form.
  5. Enter the category in space ⑦ on the application form to indicate whether or not the institution you plan to use is a member of the maternity medical care compensation scheme.
  6. If the amount paid is less than the statutory benefit amount, the difference will be paid automatically via your employer about three months after the month of childbirth. (For those covered by the system for Voluntarily and Continuously Insured Persons or the Special-Case Retired Persons Medical Care System, the difference will be transferred to the account you registered when joining the system.)

C. When you have paid the entire amount of childbirth costs to the medical care institutions

Submit the following application to the Health Insurance Association if you have not used the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institution on your behalf.

  • Copies of agreements from the medical care institution or other facility (documents demonstrating that you have not concluded a proxy agreement with the medical care institution or other facility on use of the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions and indicating the name of the Health Insurance Association as application recipient)
  • Copies of receipts for and details of childbirth expenses (Documents demonstrating that you have concluded a proxy agreement with the medical care institution or other facility to use the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions and, if giving birth at an institution that is a member of the maternity medical care compensation scheme, showing printed or stamped text certifying that the childbirth took place at a member institution)
Deadline At the earliest possible date after the birth
Applies to Insured persons or dependents who have paid the entire amount of childbirth expense at the medical care institution without using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

Caution
  1. The doctor, midwife, or other person handling the birth must fill out and affix his or her seal to the space for certification by the doctor, midwife, etc. ⑮-⑱ on the Application for Payment of Childbirth and Childcare Lump-sum Grant.
    Documents certifying the birth or similar documentation are not acceptable for this purpose.
  2. You are not eligible to receive duplicate benefits from other health insurance plans (including National Health Insurance and the Japan Health Insurance Association).
    If you plan to give birth within six months after forfeiture of eligibility under another health insurance association or other insurer under which you were covered previously, or within six months after forfeiture of eligibility under the Association, be sure to fill out completely and sign/seal spaces ⑬-⑭ on the application form.
  3. Enter a circle in the appropriate part of space ⑨ on the application form to indicate whether or not the institution you plan to use is a member of the maternity medical care compensation scheme.

D. Childbirth overseas

If you have given birth overseas, read the information at this link, then apply to the Health Insurance Association.

Add the child as a dependent

Adding a baby as a dependent requires certification by the IBM Japan Health Insurance Association.
Download the documents indicated below and submit the application form to the address shown on the upper left. When applying to add a newborn baby as a dependent, you do not need to attach documentation certifying birth. Simply submit the application form (original and copy).
If both the husband and wife are employed, the baby will become a dependent of the spouse with the higher income.
