If you lose or damage your health insurance card, etc.

Health insurance cards are no longer newly issued or reissued since December 2, 2024. Use a Myna health insurance card.

In principle, Eligibility Verification Certificates are not issued to those who have registered their Individual Number Cards for use as Myna health insurance cards.
If your Individual Number Card is lost or stolen, you will need to complete the procedures to disable the card instead of the procedures described below. Contact the Individual Number Call Center general toll-free number (0120-95-0178).

Damaged health insurance card, elderly benefits card, or Eligibility Verification Certificate

[In the case of only applying for an Eligibility Verification Certificate] (Not issued to those who have registered their Individual Number Cards for use as Myna health insurance cards in principle)

If you have lost your health insurance card, etc. and are applying for an Eligibility Verification Certificate, attach one of the following documents:

  • Copy of insured person's ID
    (e.g., driver's license, passport, employee ID)
    A copy of the insured person's ID is required even when applying for a dependent.
  • Copy of the certificate of notification issued by the police
Applies to:

Insured person or dependent whose conventional health insurance card, Eligibility Verification Certificate, or elderly benefits card is lost or damaged

[You can apply for an Eligibility Verification Certificate only in the cases specified below even if you have lost or damaged your health insurance card, etc. (In principle, no Certificate will be issued in other cases.)]

  • Lost Individual Number Card
  • In the process of renewing Individual Number Card
  • Individual Number Card electronic certificate has expired
  • Individual Number Card issued but not registered for use as a health insurance card
  • No Individual Number Card issued
  • Individual Number Card returned
  • Support from a third party (e.g., caregiver) required to receive examinations and treatment with a Myna health insurance card
  • Lost or damaged Eligibility Verification Certificate (Not including those who have registered their Individual Number Cards for use as Myna health insurance cards)

Notes:
  • The application is to be made to the employer. (Check Address inquiries to)
    (Voluntarily and Continuously Insured Persons and special-case retired persons should submit the application to the Health Insurance Association.)
  • If you find your lost conventional health insurance card, elderly benefits card, or Eligibility Verification Certificate, promptly return it to the Health Insurance Association.
  • Someone who comes across or steals your health insurance card or Eligibility Verification Certificate may use it to visit a hospital under your name. Check your medical care cost information for any costs you don't recognize.

If you lose your Certificate of Application of Maximum Copayment Amount

  • If you find your lost Certificate of Application of Maximum Copayment Amount, promptly return it to the Health Insurance Association.
  • Submit another Request for issuance of Maximum Co-payment Certificate for Health Insurance if you wish to have the Certificate of Application of Maximum Copayment Amount reissued.
