I lost my health insurance card and I'm worried someone might misuse it. What should I do?

Notify the police immediately if it was stolen (e.g., if someone walked off with it, if it was stolen from your car, or if it was stolen by a burglar). Take precautions to store your card in a secure place; there is currently no way to prevent use of a lost health insurance card. Submit an Application for Reissue of Health Insurance Card (Notice of Lost Card) to have your card reissued.
